I am not an expert in immigration issues, but I am 99% sure the answer is No.

If you are doing research the answer is yes.
For a clinical position theoretically it is also possible, but huge amount of paperwork makes it very unlikely to happen.

You should also make sure that you are not subject to the 2-year home residency requirement for certain J-1 categories. If so, you will not be able to change your status to H-1B without either obtaining a waiver or returning to your home country for 2 years.
