what are my chance?

I will be 43 y/o when trying to apply for residency in internal medicine on H visa only.

USMLE step 1 91, step 2 96

I am a board certified in internal medicine in my country
two years prior to the application completed a 3 years of fellowship at JOHNS HOPKINS BALTIMORE and have good letters of recommendation.
Have many original manuscript in good journal as first author.

What are my chances of being accepted to a program? Is my age 43 a limit?

What specialty is your John Hopkins fellowship? Did you already try to apply to the programs? With credentials like yours, I do not think age is a big problem - as long as a program knows you won't be a pain in the neck for your upper levels and attendings, which I am sure you won't.

I did a fellowship in Oncology.

Do you think I have chances to be accepted to good academic hospitals at the age of 43? I have no problems to have younger attendings. That is how the system works and I can easily comply.

You should be fine as long as programs realize there won't be any subordination problems.
